Abstract

S'han identificat dues vies carcinogèniques en el càncer de pròstata (CaPr): la positiva i la negativa per a fusions dels gens ETS. L’objectiu d’aquest estudi ha estat entendre el significat biològic i la posició jeràrquica de les alteracions moleculars responsables del CaPr. El reordenament TMPRSS2-ERG, responsable de la sobre-expressió d’ERG, caracteritza la majoria dels tumors de la via positiva per a fusions dels gens ETS. No obstant, la sobre-expressió dels altres gens ETS és un tret present en molts altres casos, i la forta sobre-expressió proteica d’ETV1 té un impacte negatiu en el pronòstic del CaPr, el qual sembla ser independent de l'estat de PTEN. En la via negativa per a fusions dels gens ETS, la pèrdua d’expressió de SPOP i PTEN podrien cooperar en la progressió del CaPr cap a graus tumorals elevats i les dues alteracions proteiques estan relacionades amb una major freqüència d’infiltració perineural. D’altra banda, les mutacions en SPOP i FOXA1 identifiquen un subconjunt de tumors ERG-negatius, amb associació amb graus tumorals elevats i recurrència bioquímica. La pèrdua de còpies del gen CHD1 està relacionada amb les mutacions de SPOP, i les dues alteracions semblen ser característiques del CaPr més avançat.
Two main carcinogenic pathways have been identified in prostate cancer (PrCa): the ETS fusion-positive and the ETS fusion-negative pathways. The aim of this project has been to clarify the biological meaning and hierarchical position of the molecular alterations along the steps of prostate carcinogenesis. The TMPRSS2-ERG rearrangement, conducting to ERG overexpression, characterizes the majority of the tumors in the ETS fusion-positive pathway. However, non-ERG ETS overexpression is a trait of a significant number of cases, and strong ETV1 protein overexpression has a negative impact on PrCa outcome that seems to be independent of PTEN status. In the ETS fusion-negative pathway, both SPOP and PTEN expression losses may cooperate in the progression of PrCa to higher tumor grades, and both protein alterations are associated with higher frequency of perineural infiltration. On the other hand, SPOP and FOXA1 mutations identify a subset of ERG wt tumors, with association with higher tumor grade and shorter time to PSA recurrence. CHD1 copy number loss is strongly associated with SPOP mutation, and both seem to be characteristic of advanced disease.
