Abstract

HER2 overexpression accounts for approximately 15-20% of breast tumors, and mainstay therapy for these patients includes HER2-specific antibodies in combination with chemotherapy. Although the efficacy of anti-HER2 antibodies has significantly improved disease control, some patients show de novo or acquired resistance to treatment, evidencing the need for biomarkers and therapeutic strategies improving patients treatment. Natural killer cells (NK) are cytotoxic innate lymphocytes specialized in the defense against virus infected and transformed cells. Several observations support the contribution of NK cells to the efficacy of anti-HER2 therapeutic antibodies in breast cancer and NK cell dysfunction, owing to immunosuppressive factors present in the tumor, has been related to tumor progression in metastatic patients. In this context, strengthening NK cell responses is envisaged as a relevant option for enhancing the therapeutic benefit of anti-HER2 mAbs. In this work we provide novel insights on the role of NK cells in the efficacy of anti-HER2 antibodies in breast cancer by describing NK cell aging as a factor potentially limiting the efficacy of anti-HER2 antibodies, and disclose the potential of CD137 costimulation for enhancing NK cell effector function despite immunosuppression.
Els tumors de mama amb sobreexpressió de HER2 correspon a un 15-20% del total de càncer de mama, i la primera línia de tractament per a aquests pacients inclou anticossos específics de HER2 en combinació amb quimioteràpia. Tot i que l’eficàcia dels anticossos contra HER2 ha millorat de forma significativa el control de la malaltia, alguns pacients presenten resistència de novo o adquirida al tractament, posant de manifest la necessitat de trobar biomarcadors i noves estratègies terapèutiques per millorar el tractament dels pacients. Les cèl·lules “natural killer” (NK) son limfòcits citotòxics del sistema immune innat especialitzades en la defensa contra infeccions víriques i cèl·lules transformades. Diverses observacions recolzen la contribució de les cèl·lules NK per l’eficàcia dels anticossos anti-HER2 en càncer de mama, i disfuncions en la funció NK degut a factors immunosupressors present en el microambient tumoral s’han relacionat amb la progressió tumoral en pacients metastàtics. En aquest context, potenciar l’acció de les cèl·lules NK es concep com una opció rellevant per augmentar l’eficàcia dels anticossos anti-HER2. En aquest treball hem aportat noves observacions al respecte mitjançant la descripció de l’envelliment del compartiment NK com a un factor potencialment limitant de l’eficàcia dels anticossos anti-HER2, i hem revelat el potencial de la co-estimulació a través de CD137 per potencial les funcions efectors de les cèl·lules NK malgrat l’immunosupressió present en l’entorn tumoral.
